Our health assessment form is designed to collect information that allows us to program and work safely with you.  Click here to fill out Health Assessment Form

  • Please do your best to fill out this form. It is at your discretion which questions you choose to answer.

  • We encourage you to answer all questions as we ask these questions for a reason - safety!

  • Wear comfortable fitness clothes. Yoga attire is fine, sweatpants, shorts, etc.

  • We do not wear shoes in the sessions.

  • Your assessment will last 15-20 min.

  • We will go over a series of general movements that help us to understand your overall mobility and flexibility levels. 

  • The program is 10 weeks long although we strongly encourage you to participate in 12 full weeks. At the end of the first 8 weeks, we can evaluate together.

  • We will go over the program structure. The first month is free. You will be charged on the 1st of the month after that. (Don't worry we will go over this and give you time before you commit to anything!)

Your in-person visit and health assessment allow us to program for you. Movements will be challenging no matter how active you are.
